Dental Warranty & Limitations

If there is a dental injury suffered while using a Shield Mouthguard, please open the Certificate of Warranty, print it, and fill it out following the directions for completing a claim form. Please keep the packaging that the mouthguard came in to identify the mouthguard model and corresponding warranty in case of injury. 

MG1

Wearer may be covered by Shield's warranty of $100.00 per injured tooth up to a maximum of $3,200.00.

MG2

Wearer may be covered by Shield's warranty of $312.50 per replaced tooth and $250.00 per injured tooth up to a maximum of $10,000.00.

MG3

Wearer may be covered by Shield's warranty of $468.75 per replaced tooth and $250.00 per injured tooth up to a maximum of $15,000.00.

MGB 

Wearer may be covered by Shield's warranty of $468.75 per replaced tooth and $250.00 per injured tooth up to a maximum of $15,000.00.
